Full Job Description
- Ensuring High-Quality Care: Ensure that staff deliver high-quality and personalised care to service users, meeting their individual needs and preferences.
- Health and Safety Compliance: Promote and maintain compliance with health and safety regulations, ensuring a safe environment for service users and staff.
- Stakeholder Liaison: Liaise with local community organisations, commissioning bodies and social care and maintain positive relationships with stakeholders to enhance support and collaboration.
- Standards Adherence: Ensure that all activities such as personal care and medication administration meet required standards and protocols.
- Reporting: Prepare reports related to patient progress and Key Performance Indicators, providing insights and recommendations for improvement.
- Resource Planning: Plan and utilise resources effectively, staying updated with key legislation and applying this knowledge to ensure the provision of the best care possible.
- Staff Management: Manage staff rotas and annual leave, ensuring adequate coverage and efficient scheduling.
- Collaboration: Work closely with operational staff to ensure that all care plans are people-focused and effective, meeting the unique needs of each service user.
- Monitoring and Supervision: Conduct spot checks and supervisions to ensure adherence to standards and identify areas for improvement or training needs.
We are seeking a detail-oriented and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the smooth operation of our service. - NVQ/QCF 2 Health and Social Care (Desirable)
- Proven experience in a similar role within the healthcare or social care sector.
- Knowledge of relevant legislation and regulations governing health and social care services.
- Strong communication and interpersonal skills, with the ability to build positive relationships with stakeholders.
- Excellent organisational and time management skills, with the ability to prioritise tasks effectively.
- Leadership qualities, with the ability to motivate and manage a team.
- Problem-solving abilities and the capacity to handle challenging situations effectively.
- Proficiency in Microsoft Office Suite and other relevant software applications.
